How do I submit a claim?

Important

Processing and lead times

All claims will be processed after the withdrawal period.
After the withdrawal period, claims will be processed within 6 working days of receipt of your documents.

Registration and refund

All your institutional fees must be paid before you can receive a refund.

Identification

We need to be able to identify you. To find out your certificate number, go to my certificate number.

Submission deadline

You can submit your claims within 365 days of the date your coverage went into effect.

6 easy ways to submit a claim.

To submit a claim by email, scan or take a picture of your invoice (dental or paramedical) using your smartphone, and email it to reclamation@planmajor.ca.

You also have the option of having your dental office submit a dental claim form directly for you, and your total bill could be instantly reduced according to the terms of your coverage. You’ll need to quote the numeric identifier TELUS ADJUDICARE 34 / AUTOBEN, your group number and your certificate number.

You also have the option of submitting your drug claims directly to the pharmacy. Please note that Plan Major is the second payer for your drug coverage, so you must first submit your claims to either RAMQ or another private insurance plan. To submit a claim, you’ll need to quote the TELUS ASSURE 34 numeric identifier, your group number and your certificate number

Claims can be submitted through our mobile app and web portal. Submission of claims and uploading of receipts can be done directly from your profile. In addition, we highly recommend that you provide your banking information under “Direct Deposit” if you wish to receive your reimbursement more quickly.

You will be prompted to create an ID using your certificate number (permanent code, student number or Major code), policy/group number, date of birth and postal code noted in your academic file.

Please ensure that your name, your certificate number (permanent code, student number or Major code) provided by your educational institution, as well as your policy/group number are included in all your correspondence with us. Please note: in order to expedite the repayment of your claims, it is highly recommended that you send us a void cheque.

Travel and accident claims

AIG

To submit a claim for accident and travel insurance, please contact the AIG Insurance Company of Canada (AIG) directly. It should be noted that there are two separate phone numbers to join them depending on whether you make an accident or travel claim.

Zurich

To file a claim for accident and travel insurance, please contact Zurich directly. Here are the steps to follow for accident and travel insurance.

Accident

When it comes to accident claims, you have 3 options:

Log on to Zurich’s electronic portal
Zurich’s electronic portal
Send an e-mail to zurichGroup@crawco.ca with your receipts and the proof required to assess your file.
By mail to the following address:
Zurich Group Claims C/O Crawford & Company, 100 Milverton Drive, Suite 300, Mississauga (Ontario), L5R 4H1. By submitting your receipts and the evidence required to assess your file.

Remember to include your name, student number and accident group number in all communications.

Please note that in the event of a claim for care received following an accident, accident coverage always applies second when you already have health coverage. If in doubt, don’t hesitate to contact us. We’ll be happy to refer you and guide you through the process.

Travel

For travel claims, please contact Zurich at the following e-mail address: travelclaims@wtp.ca
You must include your name, student number and travel group number at the beginning of your e-mail.

It is important to contact Zurich Travel Assistance prior to treatment so that they can verify certain information:
  • Confirm insurance coverage
  • Pre-approve treatment
  • Zurich can then recommend places for treatment abroad.
