Take advantage of an affordable family plan
To take advantage of an affordable family plan, you can extend your plan coverage to your children and/or spouse by completing the following form during the change period. Be sure to review the eligibility requirements under “Authorizations and Declarations” at the bottom of the form.
Be sure to visit your student association/educational institution’s page to find out all the details of your plan and the amount you will have to pay to add your dependents.
Coverage for your dependents will end on August 31. Please note that in the fall, if you are still a student and wish to maintain coverage for your dependents, you must send us this form along with your premium payment during the plan change period.
Continuing education specification:
This form allows you to extend your plan coverage to your family members (children and/or spouse) for a maximum period of 12 months. Following the initial coverage period, if you remain a student, you must send us this form along with the premium payment so that your family members can maintain their coverage.
The request must be submitted during the period when changes to your plan can be made.
Procedures
- Check the annual membership fee on your student association’s page → planmajor.ca.
- Fill out and sign the form.
- Send the form to student@majorplan.ca.
- Our team will then review your application and contact you within five business days. Wait for confirmation from a member of our team to find out the amount of the membership fee and the payment terms.
- Once payment has been made, you will receive an email confirmation within 7 business days, confirming that your application has been registered. Keep this email.
