To submit a claim for a service covered by your Major Plan, you have various options.
First, you can send it using the mobile app.
The second option is to send it by email.
To do so, scan or photograph your dental or paramedical receipt with your smart phone, and email it to us at firstname.lastname@example.org.
You must also ensure that your given name, family name, student identification number assigned by your institution and the policy number are clearly indicated at the beginning of your email.
The third option is ordinary mail.
You can submit your claims by sending all the information noted above with a copy of your receipt, or a completed dental claim form, to the address below:

You must also ensure that your given name, family name, student identification number assigned by your institution and the policy number are all clearly stated at the beginning of your letter.
Please note that whichever option you choose, if you send us a void cheque or a picture of it, we will be able to arrange payment of your refund by direct deposit into your bank account, instead of mailing it to your home address.
