Coverage period

 

The coverage period runs from September 15, 2025, to August 14, 2026, with eligibility contingent upon maintaining student status and membership in the student association.

 

Fall: August 15 to January 14

Winter: January 15 to August 14

Consent Form

 

You answered “NO” on the form regarding the transfer of your personal information during the billing process, but you still wish to take advantage of the benefits associated with your insurance plan? Here is the procedure to follow:

 

To activate your insurance coverage, please contact Plan Major at etudiant@planmajor.ca with the following information:

  • Your full name
  • Student ID (DA)
  • Educational institution

The Plan Major support team will guide you through the process. This applies only to individuals who answered “NO” on the personal information disclosure form.

 

Please note that, once your file is activated, your personal information will be shared with insurers or providers of the insurance plan for the sole purpose of ensuring the proper functioning of these services.

 

Plan Enrollment

 

The fee is billed directly to your institutional invoice. It is divided into two payments, the first during the fall semester and the second during the winter semester. In order to submit a claim and obtain a refund, please ensure that all fees on your school invoice have been paid. Below are the details of the fee for the 2025-2026 academic year.

Fall (Aug. 15 to Jan. 14) $41
Winter (Jan. 15 to August 14) $46
Total annual individual $87

 

If applicable, fees include premiums, administration fees, commissions, withholdings, and taxes that may apply to the plan.

Family Coverage

 

To take advantage of an affordable family plan, you can extend your plan coverage to your children and/or spouse during the change period.

 

Modification periods

For the fall session: Aug. 29 to Sept. 29

For the winter session: Jan. 28 to Feb 27.

Addition of child(ren) (same coverage as the insured) $87 / child
Addition of spouse (same coverage as the insured) $87 / person
