At the dentist

You can also ask your dental office to submit a dental claim form directly for you, and your total bill may be reduced instantly according to the terms of your coverage. You will need to provide them with the TELUS ADJUDICARE 34 / AUTOBEN digital identifier, your group number, and your certificate number.

At the pharmacy

You can also submit your claims for medications directly at the pharmacy. Please note that Plan Major is the second payer for your drug coverage, so you must first submit your claims to either the RAMQ or another private insurance company. To make a claim, you will need to provide the TELUS ASSURE 34 identification number, your group number, and your certificate number.

Depending on the software used by your pharmacist, you may have to provide a certificate number with at least 10 digits. Simply add zeros in front of your number to reach 10 digits.

Do you see “N/A” in your mobile app? This means that your coverage is currently inactive, so there is no need to submit a claim at this time. You may be in a transition period.
