Did you know that this plan can be added to the insurance coverage provided by your parents, spouse, and/or employer? Each plan reimburses its share to maximize your benefits.

Enrollment in the supplemental student group insurance plan is automatic; however, you may opt out at no cost during the plan’s change and withdrawal period. Plan Major will refund your premium after the change and withdrawal period ends. A $6.00 fee will be deducted from the refund for any refund request made by check.

Withdrawal Period

For the fall session: September 1 to September 30

For the winter session: February 1 to february 28

Types of Withdrawal

During the withdrawal period, you have the option to withdraw from the plan. Members enrolled in the fall session who have retained their plan cannot withdraw during the winter session withdrawal period.

The withdrawal request must be made within the prescribed time frame. You will have no other opportunity to exercise your opt-out option. To withdraw from the supplementary insurance plan, please complete and forward the Web-based form below. An email confirmation will be sent once the application has been successfully saved. Keep this email as proof of withdrawal.

On the withdrawal form, you will be asked for your certificate number, which is the same as your student ID number.
